Client Forms

New clients will need to fill out several forms before therapy can begin. To make the process as smooth and efficient as possible, the most important documents are provided here as downloads. 

Please fill out both the Intake Form and the Contact Form and bring them with you to your first session. Naturally, all forms are also available at the practice; however, filling them out in advance saves time. Additional forms will be discussed in person.
